Antithrombosis Therapy Management: Module 1 Traditional Oral Anticoagulants

Overview:
This module will cover the pharmacology, therapeutics, and drug interactions of oral anticoagulant therapy. After reviewing this module the participant will be familiar with proper indications for oral anticoagulation, goal of treatment, length of treatment, patient monitoring issues, and potential interactions with warfarin therapy.

Objectives:

  1. Discuss proper indications, duration and intensity for oral anticoagulation therapy.
  2. List proper monitoring parameters for patients receiving warfarin therapy
  3. Identify key educational elements for patients on warfarin therapy
  4. Recommend proper warfarin dosing and frequency of patient follow up
  5. List side effects associated with oral anticoagulants
  6. Identify drug-drug, drug-diet, drug-disease state interactions with warfarin.

Anticoagulation Therapy Management Certificate Program (University of Southern Indiana)

When: 6 week course - 2008 dates: March 3, April 28, June 23, August 25, and October 20.

Attendees: This program is designed for English-speaking nurses, pharmacists, physicians, and other health care practitioners who have, or want to prepare for, responsibility for monitoring and managing outpatient anticoagulation therapy.

Credit:
A Continuing Education Certificate will be awarded following successful completion of study guide questions, an online comprehensive examination, and submission of a course evaluation.

Nurses will receive 48 contact hours. The University of Southern Indiana has been approved as a provider of continuing education in nursing by the Indiana State Nurses Association (ISNA). ISNA is accredited as an approver of continuing education on nursing by the Commission on Accreditation of the American Nurses Credentialing Center. The program content meets the pharmacology continuing education requirements for the renewal of the advanced practice nurse prescriptive authority license in Indiana.

California nurses will receive 48 contact hours. The University of Southern Indiana is approved by the California Board of Registered Nursing. Provider #13378.

The Indiana Pharmacists Alliance is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmaceutical education. Participants will receive 40 contact hours or 4.0 CEU's for successful completion of this program. CE statements of credit will be distributed upon completion of the program and the program evaluation tool. ACPE # 120-999-04-201-H01. There is an additional $50.00 charge for pharmacists requesting contact hours.

Cost: $500
Fee includes Internet instruction, handouts, and a comprehensive exam. The textbook is not included in the cost of the program. There is an additional $50.00 charge for pharmacists requesting contact hours.

Description and Additional Information:
This interactive Internet Certificate Program is a six week, 40 hour continuing education program for nurses, pharmacists and physicians. Effective patient anticoagulation therapy requires systematic, coordinated patient care management by prepared anticoagulation therapy providers. This program focuses on the physiology and pathophysiology of thromboembolic disorders, patient assessment and management, pharmacology of antithrombotic agents, patient education and program administrative procedures. The content was designed utilizing the competencies for anticoagulation therapy providers developed by the Certified Anticoagulation Provider Working Group and has been reviewed by local and national experts.

Program Curriculum:
Instruction will include the following content:

  • Physiology and pathophysiology of thromboembolic disorders
  • Patient assessment and management
  • Pharmacology of antithrombotic agents
  • Patient education
  • Program administrative and reimbursement procedures

Instruction Methods:
A combination of delivery methods are used to facilitate learner and faculty communication including:

Internet
Provides on-line instructor-prepared materials and course assignments. Internet resources are used as an integral part of the instruction.

Computer Conferencing
Allows learners to interact with faculty and other course participants.

Electronic Mail
Allows learners to freely interact on an individual or group level with other learners and faculty.

Videotapes

Telephone Consultation
Faculty will be available by telephone to assist learners.

Equipment Required
Participants will need a computer with Internet access. They will also need an e-mail account. For optimum performance it is recommended that at a minimum the computer be a 486 50 Mhz or higher with 16 MB of RAM, 200 MB hard disk space, Super VGA monitor and card with a minimum of 2 MB of RAM; and a modem speed of 28,800 or higher.

The textbook for the course is Managing Oral Anticoagulation Therapy: Clinical and Operational Guidelines 2nd ed. by Jack E. Ansell, Lynn B. Oertel, and Ann K. Wittkowsky. I suggest you order your textbook as soon as possible, in order to receive it by the start of the course. The textbook currently takes 4-6 weeks for delivery. Anticoagulation Billing Certificate Program (University of Southern Indiana)

Attendees: This program is designed for English-speaking nurses, pharmacists, physicians, and other health care providers interested in the concepts and practices of reimbursement.

Credit:

The Anticoagulation Billing Certificate Program is an interactive continuing education program for health professionals. Content is delivered through the Internet, online conferencing, E-mail, and textbook readings. Participants can complete this program in the convenience of their home or office. This course is designed to be completed in three weeks and provides continuing education credits. The program has three modules. Each module includes reading assignments, study questions, case studies, and text questions. Learners interact with practitioners and faculty who are experts in the area of reimbursement. A certificate of completion will be mailed following the completion of course assignments and a comprehensive examination.

Anticoagulation Certificate Program from Univ. of Florida

Attendees: This program is intended for pharmacists wishing to increase knowledge and skills to assist in caring for patients requiring anticoagulation therapy.

Offered by: The University of Florida College of Pharmacy

Credit: The College of Pharmacy will award 3 continuing pharmacy education credits (3 CEU's) upon completion of all modules and a Certificate from the University of Florida College of Pharmacy, indicating that you have completed a rigorous course of study in Anticoagulation Therapy.

Anticoagulation Certificate Program, cosponsored by Northwest and Greensboro Area Health Education Centers and the School of Pharmacy at the University of Chapel Hill

When: Register before April 1, 2008

Where:
Participants can complete much of the program via the Internet, 9 total live hours of attendance are required at programs held in Winston-Salem. Teleconferencing opportunities may be available for the case discussion sessions for participants from other areas of the state or nation.

Overview:
The Northwest and Greensboro AHECs have collaborated with the UNC-Chapel Hill School of Pharmacy since the mid-1990s to develop and deliver highly successful pharmacy certificate programming. The certification process requires an extensive time commitment. The didactic portion of the program (approximately 7 hrs) is offered online, allowing participants to learn at their own pace. The programs also include a self-study set of readings, a pre-test, a case-based post-test, six patient interview/work-ups, a clinical services implementation plan, and attendance at nine hours of live sessions, including a skills lab and two case presentations sessions. Annual renewal programs are available for all certificates. Certificates are currently offered in anticoagulation, diabetes, hypertension, hyperlipidemia, and asthma.

The Northwest AHEC-based Anticoagulation Certificate Program has been held annually for the past six years. On average, about 15 to 20 pharmacists complete the program every year. Many have gone on to develop inpatient anticoagulation monitoring services or outpatient clinics to manage patients on warfarin.

Many pharmacists complete certificate programs to enhance their practice skills and expand clinical pharmacy services. Certification helps pharmacists meet requirements to obtain the Clinical Pharmacist Practitioner (CPP) licensure, which allows pharmacist prescribing privileges in collaborative practice with physicians. Pharmacists with training in disease-state management are increasingly developing clinics to manage complicated drug therapies. Pharmacists serve multiple roles as clinical consultants in pharmacotherapy, drug information resources, and patient educators. Certificate programs enhance the pharmacists’ expertise in monitoring drug interactions, creating individualized dosing regimens and utilizing the most cost-effective evidence-based therapies.
